Dear Friend,
We are working on a prostate cancer project on behalf of Dr. Rajiv Singal. You may be familiar with him as a prominent surgeon in the field of prostate cancer from Toronto East General Hospital.  The reason we are contacting you is because we are looking for help in order to build a handbook for partners of the men who have undergone treatment for prostate cancer.
You can help us achieve this goal by answering questions that will help us to share information with others in similar situations like yourself. Please find our questions attached to fill out and send back as soon as possible
We greatly appreciate you taking the time to help us with this project. These questions are for research purposes only and will be kept entirely confidential. All your stories will remain anonymous.

Questions for partners
1.      What was your initial reaction when you got the news that your partner had been diagnosed with prostate cancer?
2.      What information did you want right away? Where you able to get it? If so, where? If       not, what did you do?
3.      Did your partner communicate fully with you about what he was going through? If no, was this your usual level of communication or was it due to the subject matter? Did you overcome this reluctance? If so, how?
4.      Were you involved in any discussions with your partner's physician? Was this helpful?
5.      Did you discuss your partner's diagnosis with other family members? If yes, do you have any tips for others on how to do this successfully?
6.      How did you and your partner go about deciding which treatment option to pursue? What were the high and low points of your discussions? What advice would you give to other couples trying to decide which option to choose?
7.      How has prostate cancer affected:
            your partner's general physical health?
            your partner's mental health?
            your partner's quality of life (work, recreation, day-to-day activities)?
            your sexual relationship with your partner?
            your general relationship with your partner?
            your family?
8.      If you knew at the beginning of your journey through prostate cancer what you know now, what would you have done differently?
9.      What tips in general do you have for other partners of men with prostate cancer in your position?
