The best scientific evidence suggests that you should have your first PSA test in your mid 40s. After that, the frequency of testing depends on your level. The following is what doctors at Memorial Sloan Kettering Cancer Center recommend to patients with a 10-year life expectancy:
Baseline PSA test at age 45-49
