This is one of an increasing number of studies indicating that Gleason pattern 4 tissues are significantly different than pattern 3 in the risk of cancer progression. Hence the current consensus that a Gleason
6 (3+3) tumor has virtually no risk of progression compared to a Gleason 7 (3+4), and, a Gleason 7 (4+3) presents a significantly higher risk that Gleason 7 (3+4).  The challenge, of course, is obtaining an accurate biopsy (both accurate sampling AND accurate
pathology) for staging.
